Baker Student-Athletes and Parents,
The athletic trainers at Baker University, along with medical doctors from OrthoKansas, LLC, consider all student-athletes members of our family and welcome you to Baker University. We honor student-athletes by providing access to the best health care by providing evaluations, care, prevention, and rehabilitation services when athletically related injuries occur.
All information, for all sports, must be completed thoroughly and returned to the Sports Medicine Department before you are allowed to participate in any athletically related conditioning, practices or competitions. When you have completed all of the necessary forms through our online portal, please fax, email, or mail your physical exam with the provided form to the attention of the Sports Medicine Department by August 1, 2017, for beginning athletes in the fall, or January 20, 2018, for beginning athletes in the spring. In the event that the forms are dated after the above dates, there will be a $20.00 late processing fee on your student account. Please call if you have any questions.

If you already have an appointment set up for your physical within the next day or two and need access to the physical form immediately, please click here. You will still need to complete the remaining online forms through the portal before August 1. As this is a new process, there may be a few kinks; if there are any questions or trouble with the online portal, please contact Tama Crane at [email protected].
For returning student athletes, please do not include the 0’s that are before your student ID number to log in.
For new incoming student-athletes, please read the instructions thoroughly. When asked to enter your school ID #, if you do not have your school ID # please enter your cell phone number with no dashes.
